The family involved in a tragic car accident that left their 6-month-old baby dead after being rear-ended in Gates, N.Y., has been identified. Police said that their vehicle was "struck" by a black SUV.
The family of three includes Jake Carpenter, his wife Kelly Carpenter, and their 6-month-old baby Noah, who died from his injuries, Democrat & Chronicle reports, citing Gates Police Chief Rob Long.
The crash occurred on March 5 at approximately 10:15 a.m. local time, the Gates Police Department said in a press release.
"The Gates Police Department responded to the intersection of Buffalo Road and Howard Road for the report of a multi car motor vehicle accident. Information was relayed through dispatch that people in the two vehicles may be trapped," police said.
At the scene, police determined that it was a "two-car accident with entrapment." Gates Fire and Gates Ambulance also provided emergency response to the victims.
"Upon Gates Fires' arrival they were able to successfully remove individuals from two cars which sustained heavy damage," police said.

"From the initial investigation we learned that the driver of a black SUV struck the rear of a white SUV stopped at the Buffalo Road traffic light facing eastbound at the Howard Road intersection," Gates police continued. "There were (3) occupants in the white SUV. A driver, front seat passenger and 6-month-old child in a child safety seat."

All three members of the Carpenter family were immediately hospitalized for their injuries.
"Unfortunately, due to the injuries sustained, the 6-month-old child passed away while at Strong Memorial Hospital," police said, adding that both Jake and Kelly are in "critical condition."
"Kelly has suffered a severe traumatic brain injury requiring emergency surgery," the description of a GoFundMe started for the family by Laura McGlynn reads. "Jake suffered a serious injury to his aorta and also needed emergency surgery. Jake is expected to make a full recovery, but Kelly remains in critical condition."
The cause of the crash remains under investigation. PEOPLE has reached out to the GPD for further information.
